Fatty liver disease is becoming more common today because of unhealthy eating habits, stress, and lack of exercise. Many people do not know they have fatty liver until the condition becomes serious. But according to Dr. Brijmohan Arora, a senior diabetologist from Dwarka with 24 years of experience, there are some physical signs that may help identify fatty liver and insulin resistance even without a blood test.

Dr. Arora shared this information in an Instagram video, where he talked about a patient who did not want to do any blood tests but wanted to check the health of his liver. The doctor explained that there are clear signs on the body that can indicate if a person may have fatty liver or is moving toward pre-diabetes.

One of the main signs Dr. Arora discussed is hard and large belly fat, also known as visceral fat. This type of fat forms deep inside the abdomen around important organs. He said that if the stomach feels very firm and is sticking out, it can be a strong sign that fat is building inside the liver.

Another sign is the presence of skin tags—small, soft skin growths that often appear around the neck or underarms. Dr. Arora explained that skin tags are a common signal of insulin resistance. Insulin resistance means that the body is not using insulin properly, which can lead to diabetes and fatty liver.

He also pointed out red or purple lines on the lower legs, especially around the ankles. These lines can show that blood vessels are under pressure, which can be linked to poor metabolic health and insulin-related problems.

A puffy or swollen face is another sign. If a person’s cheekbones are not visible anymore and the face looks round or bloated, it may suggest fat is building inside the liver.

Dr. Arora also mentioned high blood pressure, such as readings above 140 or 150, as a very important clue. He said high blood pressure is often connected to insulin resistance, even before diabetes is diagnosed.

The doctor shared that fatty liver and insulin resistance work like a harmful cycle. When fat builds in the liver, insulin resistance increases. In turn, insulin resistance causes even more fat to collect in the liver. If ignored, this condition can lead to serious liver disease, including NASH, which involves liver inflammation and long-term damage.

While these physical signs can be helpful, Dr. Arora made it clear that blood tests and medical check-ups are still important for confirmation. These signs are warnings. If someone notices them, they should consider making changes to their lifestyle and consult a doctor.

To reduce the risk of fatty liver, Dr. Arora suggests simple habits such as eating fewer sugary foods, avoiding junk food, exercising regularly, sleeping well, and keeping stress under control. Drinking water and including natural fiber in meals can also help the liver work better.

Fatty liver disease can be reversed in many cases if detected early. The key is paying attention to the body, taking small steps toward healthier habits, and seeking medical advice when needed.

This report reminds us that health is often reflected in small physical changes. The body speaks—we just need to listen.
