Marissa Dawson, a 35-year-old mother residing in Moncton, was prescribed an antihistamine for chronic eczema. Instead of the correct medication, she mistakenly received a drug intended for high blood pressure treatment. Unaware of the error, Dawson consumed the wrong drug for months, suffering from dizziness, breathing issues, and severe fatigue.
Her health deteriorated, leading to an emergency room visit where medical professionals uncovered the error. The shocking revelation left Dawson worried about the potential consequences if the medication had been given to a child.
An investigation by the New Brunswick College of Pharmacists indicated that the mistake stemmed from confusion between similarly named medications and staff fatigue. Additionally, a major procedural oversight was identified—the lack of pharmacist counselling during the prescription pick-up. This crucial step is vital in catching mistakes before they affect patients.
The pharmacy has acknowledged the error and initiated corrective measures, including comprehensive staff training, clearer operational protocols, and enhanced reminders for frequently confused drug names. They have also reinforced the need for counsel during the dispensing of new prescriptions.
With Canada issuing over 800 million prescriptions yearly, the full scope of medication errors remains uncertain due to inconsistent error reporting between provinces. National databases only report a fraction of incidents, indicating that the actual number of errors could be substantially higher.
Experts assert that these incidents point to broader systemic issues rather than isolated errors. Rising workloads, intricate healthcare networks, and multiple care points have complicated the maintenance of reliable safety standards. Researchers advocate for robust systems, enhanced error tracking, and improved communication among healthcare practitioners.
Advocates are pushing for reforms, including clearer drug labeling, advanced technology utilization, and mandatory error reporting protocols. Several provinces are also formulating policies aimed at addressing pharmacist fatigue and enhancing work conditions.
